Why do antibiotics stop working?

The Hook

Antibiotics are losing their power — not because the drugs changed, but because bacteria evolve, fast.

The Science

An antibiotic kills most of the bacteria in an infection — but by chance, a few may carry a trait that lets them survive: a pump that ejects the drug, an enzyme that chops it up, or a slightly changed target the drug can’t grip. When the others die, those survivors are left to multiply, and now the whole population is resistant.

Bacteria speed this up by swapping resistance genes directly with each other, even across species. Many of these genes ride on small loops of DNA called plasmids that pass from one bacterium to the next like a shared file — so a harmless gut microbe can hand a dangerous pathogen the instructions to survive a drug. Every use of an antibiotic — especially unnecessary ones — nudges this selection along.

It’s worth being clear about what antibiotics can’t do: they don’t work on viruses at all. Taking them for a cold or the flu does nothing to the virus but still pressures the harmless bacteria living in and on you to evolve resistance — a cost with no benefit. Resistance also mostly isn’t something the drugs “create” — the genes were largely already out there (though sub-lethal doses can nudge bacteria into generating new ones). Antibiotics just clear the field so the few resistant survivors can take over.

  • Resistance is ancient. Scientists have found antibiotic-resistance genes in bacteria frozen in 30,000-year-old permafrost, long before humans made a single drug.
  • Alexander Fleming, who discovered penicillin, warned about resistance in his 1945 Nobel lecture — the problem was predicted almost from the start.
  • Bacteria can pass resistance across completely different species, so overusing one antibiotic can quietly breed resistance in bugs it was never even aimed at.

Why It Matters

Resistance is turning once-trivial infections back into serious threats and endangering surgery, chemotherapy, and childbirth. It’s the clearest everyday example of natural selection happening in days — and why not overusing antibiotics genuinely matters. The drugs still work; the job is to keep them working.
