What actually happens during a stroke?

The Hook

Your brain is an energy hog with almost no reserves — it needs a constant flow of oxygen-rich blood, second by second. A stroke is what happens when that flow to part of the brain is suddenly cut off. The affected neurons begin to die within minutes, which is why stroke is one of the most time-critical emergencies in medicine.

The Science

There are two main kinds, and they’re opposites:

  • Ischemic stroke (the majority): a blockage — usually a blood clot — plugs an artery feeding the brain, so the tissue downstream is starved of oxygen.
  • Hemorrhagic stroke: a blood vessel in the brain bursts and bleeds, both depriving tissue of normal flow and damaging it with pooling blood and pressure.

Either way, the starved brain region can’t function, and it can’t wait. Neurons denied oxygen start dying fast, and dead brain tissue generally doesn’t grow back. Whatever that region controlled — speech, movement of one side, vision — abruptly fails.

That’s why the warning signs spell F.A.S.T.: Face drooping, Arm weakness, Speech difficulty, Time to call emergency services. The treatments — clot-busting drugs or physically removing a clot — only work within a narrow window, so hours literally decide how much brain is saved.

Here’s the part that makes speed so valuable. Around the dying core of an ischemic stroke sits a rim of tissue called the penumbra — brain cells that are starving but not yet dead, kept barely alive by trickles of blood from neighboring vessels. Restore the flow in time and much of that rim can recover. Wait too long and it joins the dead core. Treatment is essentially a race to rescue the penumbra before it’s lost.

There’s also a temporary version worth knowing: a transient ischemic attack, or “mini-stroke,” where a clot briefly blocks flow and then clears on its own. The symptoms vanish within minutes to hours — but it’s a serious warning sign that a full stroke may be coming, and it should never be shrugged off.

Three Wild Facts

  • Doctors say “time is brain” because during a large ischemic stroke the brain can lose roughly 1.9 million neurons a minute while flow stays blocked.
  • A transient ischemic attack can mimic a full stroke and then vanish within an hour — but it’s a red flag that a bigger stroke may follow, sometimes within days.
  • Giving a clot-buster to the wrong type of stroke can be deadly — the same drug that rescues a blockage would worsen a bleed — which is why a brain scan comes before treatment.

Why It Matters

“Time is brain.” Recognizing a stroke and calling for help immediately is the single biggest factor in the outcome, because every minute of blocked flow means more neurons lost. Understanding the two types also explains why treatment must be exact — a clot-buster that saves an ischemic stroke would be catastrophic for a bleeding one, which is why imaging comes first.
