How does cancer hijack the nervous system?

The Hook

For a long time, nerves near a tumor were treated as innocent bystanders. It turns out some cancers actively pull nerves in and use them — turning the nervous system into part of their own support system.

The Science

There are two connected ideas here.

The first is perineural invasion. Some tumors — pancreatic, prostate, and head-and-neck cancers especially — grow along the outside of nerves, using them like a highway to travel through the body. Where this happens, patients often have more pain, and it tends to signal a more aggressive cancer.

The second is newer and stranger, from a field called cancer neuroscience. Just as tumors recruit blood vessels to feed themselves, researchers have found they can recruit nerves. The tumor releases nerve-growth signals (like nerve growth factor), coaxing nerve endings to sprout into it. In return, those nerves release neurotransmitters — the same chemical messages your brain uses — that spur the cancer to grow. In some brain tumors, cancer cells have even been shown to form real, synapse-like electrical connections with healthy neurons.

So it’s a two-way conversation: the tumor calls nerves in, and the nerves help it thrive.

There’s a useful parallel here. Decades ago, researchers realized tumors can’t grow large without recruiting their own blood supply — a process called angiogenesis — and that insight led to a whole class of cancer drugs. Cancer neuroscience is asking the same question about nerves: if a tumor needs to be “wired in” to grow, then the wiring itself might be a weak point. The nerves involved are often part of the autonomic nervous system, the automatic network that also controls things like heart rate and digestion, and in some cancers the balance of these nerve signals seems to push tumor growth in one direction or slow it in another.

It’s worth being clear about the state of the science: this is an active, fast-moving field, and much of the detailed mechanism is still being worked out. What’s solid is the big picture — nerves are not passive bystanders around a tumor, and the connection runs both ways.

Three Wild Facts

  • Perineural invasion — cancer creeping along the outside of nerves — is one reason some tumors are so painful and so hard to fully remove: the cancer can travel down a nerve well beyond the visible edge of the tumor.
  • In lab studies of some brain tumors, cancer cells were found forming real synapse-like connections with healthy neurons and responding to the brain’s own electrical activity — essentially eavesdropping on the circuits around them.
  • The idea has a striking clinical hint: studies have observed that patients on certain nerve-blocking medications, like some blood-pressure drugs, sometimes show differences in how particular cancers behave — a clue that’s now being explored deliberately.

Why It Matters

If nerves are actively feeding certain cancers, then the nervous system becomes a target. It’s part of why researchers are testing whether drugs that block specific nerve signals — including some existing medications — could slow tumors down. It also reframes cancer: not a lump growing in isolation, but something wired into the body’s own signaling.
