What goes wrong in the aortic valve — and how do you fix a valve inside a beating heart?
The Hook
Every heartbeat sends blood out through a single one-way door: the aortic valve. It opens to let blood leave the heart, then snaps shut so none flows back. When that door stiffens and won’t open fully, the whole body feels it — and the modern fix is almost hard to believe.
The Science
The aortic valve sits between your heart’s main pumping chamber (the left ventricle) and the aorta, the great artery that feeds the body. Its thin flaps (leaflets) open and close roughly 100,000 times a day.
The most common problem is aortic stenosis — the valve narrowing and stiffening, often from calcium building up on the leaflets over decades. Now the heart has to shove blood through a partly jammed door, so it strains, thickens, and eventually can’t keep up. Symptoms are chest pain, breathlessness, and fainting.
A healthy aortic valve normally has three leaflets — like three little cups that meet in the middle and seal tight. Some people are born with only two (a bicuspid valve), and that slightly-off design tends to wear out faster, which is why stenosis can show up decades earlier in them. The other direction of failure is a valve that won’t close properly (regurgitation), so blood leaks backward and the heart has to re-pump the same load. Either way, the muscle pays the price.
For a long time, fixing it meant open-heart surgery: stopping the heart, putting the patient on a machine that pumps their blood, and sewing in a new valve. Today there’s often a gentler option — TAVR (transcatheter aortic valve replacement): doctors thread a collapsed replacement valve up through a blood vessel (usually in the groin) on a thin tube, position it inside the old valve, and expand it in place — all while the heart keeps beating. No cracked-open chest. The new valve is often built from treated animal tissue (cow or pig), stitched onto a small metal frame that anchors it against the old leaflets.

- That familiar “lub-dub” heartbeat is largely the sound of valves slamming shut; the “dub” is your aortic and pulmonary valves closing at the end of each beat.
- The very first replacement heart valves, in the 1950s and 60s, used a tiny silicone ball trapped in a metal cage — patients could sometimes hear their own valve clicking.

The aortic valve is a vivid case of the heart as plumbing under enormous, relentless load, and of how far medicine has come — repairing a structure deep inside a living, beating heart through a pinhole. For many older or high-risk patients, TAVR turned a formerly major operation into a same-week recovery.
